David Jacofsky, MD
Chairman & Chief Executive Officer David J. Jacofsky, MD is an international speaker and respected authority in complex adult joint reconstruction, total joint replacement, traumatology and oncology. He is committed to delivering the finest patient care possible, community service, research and education. Dr. Jacofsky has extensive background in business management. He chairs the Orthobiologics Committee for Stryker Orthopedics, is a Board Member for Cold Plasma Medical Technologies, has been the CEO of multiple successful start-up companies, and has overseen The CORE Institute’s strategy and growth since inception. Dr. Jacofsky is on the Editorial Board of The Journal of Arthroplasty, Clinical Orthopedics and Related Research, and Orthopedics.
Dr. Jacofsky continues his career as the Chairman of The CORE Institute after a successful tenure as a Division Director and Assistant Residency Director at the Mayo Clinic in Rochester, Minnesota. He brings to The CORE Institute the best of what Newsweek magazine hails as “the top rated Orthopedic Department in the United States 12 years running.” The Arizona Business Magazine recognized him at their Healthcare Leadership awards two years running, receiving the Special Merit Award for Innovation in 2013 and the Surgeon of the Year Award in 2012. In addition, he received “Leaders in Joint Replacement” Award from OrthopedicsToday in 2013 and was recognized as PHOENIX Magazine’s "Top Doc” in 2010. He also received the 2013 Health Care Leadership Award from AZ Business Journal.
His career landmarks include graduating Valedictorian of his high school class, highest honors at Lehigh University, valedictorian and magna cum laude at the Medical College of Pennsylvania, orthopedic surgery residency at the Mayo Clinic in Rochester, Minnesota, and finally a fellowship in orthopedic oncology and complex adult reconstruction from Johns Hopkins University in Baltimore, Maryland. During his medical education he received numerous awards including the 2001 Mayo Scholar award and the Joe Janes Humanitarian Award Dr. Jacofsky was named Surgeon of the Year by Phoenix Business Journal in 2012 and received an award for being a leader in innovation in health care from the same magazine in 2013.
Dr. Jacofsky is extensively involved with academic medicine. To date, he has 41 peer reviewed publications, 19 book chapters and three (3) orthopedic textbooks to his credit. He routinely lectures at local, regional, national and international events. He is board certified by the American Academy of Orthopaedic Surgeons.
Damon Adamany, MD
Damon Adamany, MD is a board certified and fellowship-trained specialist in hand and upper extremity surgery and is certified in basic microsurgery with The CORE Institute.
Dr. Adamany completed his fellowship training at the University of New Mexico in hand and upper extremity surgery with advanced microvascular training. Prior to his fellowship, he completed his residency at Mount Carmel Medical Center in Ohio, where served as Vice President of the Mount Carmel Residency Forum, addressing issues that affect young physicians, while developing a curriculum for multidisciplinary grand rounds. Dr. Adamany also completed a Pediatric Orthopedic surgery rotation at Columbus Children’s Hospital in Ohio. In 2003 he received an additional certification in Basic Microsurgery from the Cleveland Clinic.
Dr. Adamany earned his medical degree at the University of Arizona, where he graduated in the top quarter of his class. He obtained his bachelor’s degree, magna cum laude, in physiological sciences at the University of Arizona. He has maintained a strong commitment to serving the Arizona community through his hundreds of hours of volunteer work in the emergency department at Tucson Medical Center and at rural pediatric orthopedic clinics, most notably with the organization Commitment to Underserved People. Dr. Adamany also participates in The CORE Institutes Community Lecture Program which provides the public informational presentations regarding orthopedic care.
Dr. Adamany has written and contributed several articles in academic journals, including the Journal of Hand Surgery. He has also given numerous professional presentations as well as a poster presentation for the American Academy of Orthopaedic Surgeons.
Charlie Agee, MD
Vice President Clinical Stewardship
Arizona West Region Chief Medical Officer
Charlie M. Agee, MD is the Chief Medical Officer of the Arizona West Region for Banner Health with direct supervisory responsibility for all of the chief medical officers in the region. He has leadership responsibilities with the Care Management executive team and serves on other systemwide teams for Banner Health. He leads the systemwide clinical strategic initiative teams for the Surgical Care Improvement Project as well as Reducing Variation in Orthopedics. He also serves on the board of Banner Core Center for Orthopedics. Dr. Charlie Agee continues in his role as CMO of the Arizona West Region and also carries the title of Vice President of Clinical Stewardship. In addition to his current responsibilities for Physician Care Coordination, he assumes responsibility for all of Banner Case Management functions as well as clinical supply chain management.
In his position Dr. Agee has a key role in linking evidence and consensus-driven medicine with clinical operations to ensure that safe, reliable care is delivered. He also has responsibility for developing innovative models of care that deliver value and maximize patient’s wellness.
Prior to his current position, Dr. Agee was the CMO for Banner Del E. Webb and served as the first CMO for Banner Estrella Medical Center. There he was instrumental in developing and overseeing the Professional Review Committee which developed a new peer review program. He also led the development of medical staff bylaws there and opened Banner’s first fully Electronic Medical Record facility.
During his 18-year career in obstetrics and gynecology, Dr. Agee served as Chief of Staff for Banner Thunderbird Medical Center. He is a graduate of the University of Arizona, attended medical school at the University of Washington in Seattle and completed his Obstetrics and Gynecology residency at Maricopa Medical Center in Phoenix.
Eric Tomlon, FACHE, MBA/MSHA
Eric Tomlon currently serves as the Chief Operating Officer for Excellence Headquarters/The CORE Institute and has been an instrumental member of the Banner CORE Center for Orthopedics management team. He has more than 20 years of experience in health administration.
Eric obtained his MBA and Masters of Science in Health Administration from the University of Colorado at Denver. He is a Fellow in the American College for Health Care Executives, and Member of the Medical Group Management Association. His experience includes seven years of executive level hospital administration and ten years of executive physician group administration. Areas of experience include operational leadership, medical staff leadership, human resources, strategic and operational planning, and physician/hospital alignment strategies. His career has included positions with for-profit, not-for-profit, Children’s hospitals, Catholic, academic and independent health systems.
Eric has served on all types of community organizations including the Regent’s Advisory Council for the Arizona and Colorado Chapter for the American College of Health Care Executives, Colorado Health Administration Alumni Association, University of Colorado Health Administration Student Organization, Rotary, Greater Phoenix Economic Council and other community organizations.
Debbie Flores is the CEO of Banner Del E. Webb Medical Center and brings years of healthcare leadership experience to the role. Previously, she had been CEO of Banner Heart Hospital and associate administrator at Banner Boswell Medical Center. Flores is a seasoned health care executive who has served in a series of increasingly responsible management positions over the last 29 years. Before joining Banner Health in 2006, Flores was the chief operating officer at Paradise Valley Hospital, West Valley Hospital and Phoenix Baptist Hospital. She started her career in health care in 1981 as a Human Resources professional. Under her leadership, the hospital celebrated its 25th anniversary in 2013 and the opening of new state-of-the-art heart catheterization and endoscopy labs. The LeapFrog Group named Banner Del E. Webb one of the nation’s top hospitals for 2013.
Scott P. Leckey, CPA
Chief Financial Officer
Banner Desert and Cardon Children’s Medical Center
Scott Leckey has more than 20 years of health care administration experience in both for-profit and non-profit health systems. He currently serves as the chief financial officer for Banner Desert Medical Center and Cardon Children’s Medical Center in Mesa, Ariz., one of non-profit Banner Health’s largest acute care medical campuses. He has served in similar capacities within Banner Health since 2010, first serving as chief financial officer for Banner Estrella Medical Center in Phoenix and, more recently, as chief financial officer for Banner Boswell Medical Center in Sun City, Ariz.
As CFO at Banner Boswell, Mr. Leckey was part of the senior operations team that effectively completed the hospital’s transition from Sun Health to Banner Health, which acquired Sun Health’s hospitals in 2008. During his tenure at Banner Boswell, Mr. Leckey also helped oversee the implementation of a computerized physician order entry system and electronic medical record system.
Prior to joining Banner Health, Mr. Leckey served in chief financial officer positions with several other health systems, including Hawaii Health Systems, Iasis Healthcare and Tenet Health System.
He received his undergraduate degree in accounting at Loyola Marymount University in Los Angeles, and is a member of the American College of Healthcare Executives and American Institute of Certified Public Accountants. He serves as treasurer of Banner Federal Credit Union and as a board member and volunteer with the Chandler Chamber of Commerce and Habitat for Humanity of Central Arizona.
Outside of his professional and volunteer work, Mr. Leckey actively participates in Ironman events as a triathlete, finishing events in 2012 and 2013.
